- A list of existing Employee Safety Representatives (ESRs) and/or Health and Safety Representatives (HSRs) including when they were last elected, what training they have had and when, and which work area/group they represent Tuesday 5 February, and
- A list of existing Health and Safety Committees, howsoever named, including the name of the chairperson and which work area(s)/group(s) they represent by Tuesday 5 February.
